Understanding the "Physicians with Canadian Work Experience" Stream
For physicians already practicing in Canada, 2026 offers one of the most favorable Express Entry environments in recent years. Immigration, Refugees and Citizenship Canada (IRCC) has launched a specialized category specifically for “Physicians with Canadian work experience,” allowing qualified doctors to receive Invitations to Apply (ITAs) even when their CRS scores might not be competitive in general draws.

What is the “Physicians with Canadian Work Experience” Category?
This is a category-based selection stream. IRCC can now issue invitations to candidates in the Express Entry pool who meet specific physician criteria, rather than just ranking them by total points.
How it works: You must still be eligible for a base program (CEC, FSW, or FST) and meet the specific physician category requirements.
What it is NOT: It is not a medical license, it is not a permanent fixture (IRCC can change categories at any time), and it does not waive the basic eligibility rules of Express Entry.
Strategic Note: If you do not yet have 12 months of Canadian experience, your focus should be on qualifying for the Federal Skilled Worker (FSW) program while simultaneously building your Canadian work history and licensing plan.
Critical Changes in 2025–2026
Two major policy shifts have fundamentally changed how doctors should approach their profiles:
Job Offer Points Removed (March 25, 2025): IRCC no longer awards CRS points for job offers. Having a contract will help with certain provincial pathways, but it will no longer give you the 50–200 point boost in the Express Entry pool.
Upfront Medical Exams (August 21, 2025): Applicants must now complete their immigration medical exam before submitting their PR application. You can no longer wait for IRCC to request it after you apply.
